Types of Bipolar Disorder
Bipolar disorder is a mood disorder characterized by episodes of elevated mood and episodes of depression. The nature and severity of these episodes determine the specific diagnosis.
Bipolar I disorder involves manic episodes that last at least seven days or require hospitalization. Manic episodes include elevated or irritable mood, increased energy, decreased need for sleep, racing thoughts, and impulsive behavior. Depressive episodes also occur but are not required for diagnosis.
Bipolar II disorder involves hypomanic episodes, which are less severe than full mania, along with major depressive episodes. Hypomania may feel productive and energizing, but it still represents a departure from normal functioning and often precedes depression.
Cyclothymic disorder involves chronic fluctuating moods with periods of hypomanic and depressive symptoms. These symptoms do not meet the full criteria for manic, hypomanic, or major depressive episodes but persist for at least two years.

Manic, Hypomanic, & Depressive Episodes
Manic episodes involve a distinct period of abnormally elevated, expansive, or irritable mood. During mania, individuals may feel invincible, take on excessive projects, spend money recklessly, or engage in risky behaviors. Sleep need decreases dramatically, and thoughts race faster than speech can follow.
Hypomanic episodes share similar features but at reduced intensity. The person may feel unusually good and productive. Others may notice the change, but functioning is not severely impaired. However, hypomania often signals an impending mood shift and requires attention.
Depressive episodes in bipolar disorder resemble those of major depression. Symptoms include persistent sadness, loss of interest, fatigue, sleep changes, appetite changes, and thoughts of death. Bipolar depression can be particularly severe and carries significant risk.
Importance of Early Diagnosis
Bipolar disorder often goes undiagnosed or misdiagnosed for years. Many individuals first seek help during depressive episodes and receive a depression diagnosis. Without recognizing the bipolar pattern, treatment may be ineffective or even counterproductive.
Early and accurate diagnosis allows appropriate treatment to begin. With proper care, individuals with bipolar disorder can significantly reduce the frequency and severity of episodes. Delayed diagnosis typically means more episodes, more impairment, and more difficulty achieving stability.
Therapy Approaches for Bipolar Disorder
Therapy plays a vital role in bipolar disorder treatment alongside medication management. At Southside DBT, we use evidence-based approaches to help you manage mood episodes and maintain stability.
CBT for bipolar disorder helps you identify warning signs of mood episodes, challenge thoughts that maintain problematic moods, and develop strategies for managing symptoms. Behavioral interventions address sleep regulation, activity scheduling, and stress management.
DBT provides skills for emotional regulation that help moderate mood swings. Mindfulness practices increase awareness of mood changes, allowing earlier intervention. Distress tolerance skills help you cope during difficult periods without making impulsive decisions.

Emotional Regulation & Lifestyle Stability
Lifestyle factors significantly influence bipolar disorder. Irregular sleep schedules, high stress, and substance use can trigger episodes. Treatment includes developing routines that support mood stability, including consistent sleep and wake times, regular exercise, and stress management practices.
Emotional regulation skills help you recognize and respond to mood shifts before they escalate into full episodes. You learn to identify early warning signs and implement intervention strategies. Over time, these skills become automatic and provide ongoing protection.
Relapse Prevention
Bipolar disorder is a chronic condition that requires ongoing management. Therapy helps you develop a relapse prevention plan that identifies your personal warning signs, specifies actions to take when symptoms emerge, and involves supporting people who can help recognize changes you may miss.
